Access your Electronic Health Record

Once you enroll, you will have access to your health record through your patient portal. This is a secure way to communicate with Dr. Myhre through your Patient Passport, as well as see your lab results, imaging results, and specialist reports.

Clinic Forms

At your first visit, you will be given a paper copy of the forms below if requested. However, they are listed here for your convenience.

  • Patient Agreement
  • Clinic Policies
  • Notice of Privacy Practices
  • Release of Information (for you to get your records from other doctors and providers).
  • HIPAA Release of Information (who do you want protected health information to be released to and way you prefer to communicate with the practice)
  • New Patient forms will be emailed to you once you schedule an appointment. What you type in these forms will upload into your electronic medical record. (If you would rather not use the internet, Dr. Myhre can mail you a copy of the forms).
  • For Osteopathic Manipulation visits, Dr. Myhre will email (through your Patient Passport) you a form before your visit to fill out regarding your musculoskeletal concerns/needs. These can be mailed to you as well.
  • A Pre-Visit form will also be emailed (through your Patient Passport) before regularly scheduled visits. This will save us time at the beginning of the visit. Filling this out before your visit will let Dr. Myhre know what concerns you would like addressed at that visit. If you do not want to fill this out before your visit, that is not a problem.

How do I contact Dr. Myhre?

  • If it is an emergency, DO NOT contact Dr. Myhre through your electronic health record or call Dr. Myhre.  If you really think you might be experiencing a life-threatening emergency, call 911 or go to the nearest Emergency Department.  Once you are on your way to the ER or are already there, you can call or text Dr. Myhre to let her know.  Whenever possible, she can talk to the ER doctors and staff to share important medical information about you if it will help improve your emergency/hospital care.
  • For all non-urgent communication about your health, and to schedule appointments, please log in to your Patient Passport in your electronic health record. Dr. Myhre will get back to you in 24 hours (weekdays).   If preferred, you can text or leave a voicemail at (360) 322-1281.  Please DO NOT email Dr. Myhre for health-related concerns. Email is not secure and thus not HIPPA-compliant.
  • For any urgent questions, please call (360) 322-1281 and press “1” to leave an urgent message.   She checks voicemails and all other communication at 9:00 AM on weekdays and typically responds to urgent messages within 3 hours.  For active patients, you can use secure text messaging at the same number, which you access through the Spruce app on your smart phone. (This is a free download).  For those without a smart phone, you can still text message me at (360) 322-1281. It would be like sending any other text message and is therefore not considered HIPAA-compliant in terms of privacy laws. 
  • All calls and text messages do get forwarded directly to Dr. Myhre’s personal cell phone so that she can be as accessible as possible.  All text alerts are turned off at night but her ringer will always be on.  Please avoid calling at night or early in the morning unless it is urgent.  She will check emails, electronic health record communication, voicemails and texts at 9:00 AM weekdays.  
  • If Dr. Myhre will be out of cell range she will make this clear on her voicemail.  If she is to be out of cell range for longer than a weekend or during business days, she will let her patient’s know ahead of time by email and/or letter.
  • For urgent matters that occur when Dr. Myhre is not available, please go to one of the below local urgent care facilities:

What to expect at your visits

  • Your first visit, whether you have seen Dr. Myhre at OMC or not, she will review the Patient Agreement (this is required by the State of Washington and is found on the enrollment website).  You may have already signed this document when you first signed up online. She will review this again during the first visit. Dr. Myhre will review your New Patient History Form (unless you were Dr. Myhre’s patient at OMC). If possible, please fill out this form before your visit online, and it will be automatically uploaded into your patient portal.  If it is not possible to fill it out online, Dr. Myhre can complete and review it with you at your visit. After these forms are reviewed, you will be able to talk with Dr. Myhre about your health care needs/goals. At the conclusion of the visit, follow up visits will be scheduled. This process will take 60-75 minutes.  If you were previously Dr. Myhre’s patient at OMC, she will do her best to pick up where you left off at your last visit with her at OMC.
  • Your follow up visits are typically 30-45 minutes long. For established patients, follow up might be quicker since Dr. Myhre will know you well. Some issues could be addressed over the phone or texting as appropriate. If needed, TeleVisits can be done (on the phone or video).
  • When you arrive, please park in the parking-lot behind the building. Dr. Myhre will meet you there to let you in the back door. Please wear your mask. Please do not enter the front door and wait in the waiting room. In light of COVID-19, we would like to decrease the amount of patients in one space at a time so will not be using the waiting room at this time. There is wheelchair access from the back parking-lot as well. Dr. Myhre will ask you COVID-19 screening questions and taking your temperature in the parking lot.
  • When you leave, you will have a summary of your visit that you can view on your Patient Passport. If you prefer a paper copy, Dr. Myhre can print it before you leave.
